At Improving Me we use evidence based information only (1), and that is the knowledge we share with you. We promote healthy lifestyle initiatives.
HOW WE WORK: We see you as a whole. With your previous condition/s and the newly diagnosed, with your priorities in life, activities, jobs, things that need to happen for you, what makes you happy and what not. With this in mind create a plan and goal that are doable and achievable for YOU.
GETTING TO KNOW EACH OTHER: We offer a first, free of charge, 30 min telephone session to make sure our services are what best suits your needs at this present time. When we decide to move forward together there are different ways you may want to work, in person at your place, in our office, at a local Cafe or by skype or phone, either weekly or bi-weekly.
FIRST INTEGRATIVE HEALTH ASSESSMENT: We will perform an integrative health assessment (medication review and reconciliation, natural health products, vitamins, homeopathic medicines; BMI calculation, sleeping pattern, health concerns, etc)
DISCOVERY SESSION: What is your learning style? What are your goal/s? Do you have a timeline in mind?
ACTION PLAN AND START: We will create and agree on an action plan and attentively coach you to achieve your goals, to help you go from ambivalence to change, transforming plans into actions.
TRACKING AND ACCOUNTABILITY: With weekly or bi-weekly face to face, telephone or online coaching sessions we will track your improvement and discuss the progress you are making to better understand its value. You are accountable for your own accomplishments and we will be available for you in the process.
During our coaching sessions, you will be provided with evidence based information that is relevant to you and that aligns with your Action Plan and Goal/s. Those tools serve to make changes sustainable.
SUMMARY: We will compile your personalized medical and wellbeing history, to share the information with other healthcare professionals you choose or for your own records.
Rates are based on the type (in person or online) and amount of sessions agreed upon (3 session pack, 30-day challenge packages available).
If you have private insurance, please check with them as most insurance covers Health Coaching services when provided by a qualified Registered Pharmacist.
