A prescription at discharge (PDF)
Compliance:
Studies show 1 in 5 patients do not take their medications as prescribed. Moreover, they don’t fill out the Prescription they are given.
Dr. Saposnik (St Michael’s Hospital) conducted several studies showing that at least 20% of high risk patients are not in target values for cholesterol blood work 1 year after intervention.
Evidence from studies conducted in Canada, USA and Europe show that a large group of patients fail to achieve guideline-recommended targets, and this apparent treatment gap was more pronounced among patients with cardiovascular disease and women. Quality improvement strategies should target these patient subgroups**.
To add more, a 2016 study published in Hypertension revealed that pharmacist intervention reduced racial and socioeconomic disparities in the treatment of blood pressure.***
Only 67% of your patients will fill your Prescription after a week of being discharged, and data shows that among them 12% will not fill or renew the Prescription after a year. That number drops another 5% at the 2 year mark*.
